Vaccination Exemptions:

Personal Belief Exemption Form (Kindergarten - 12th Grade)   -   

If you are submitting an exemption form for your k-12th child for school, utilize this link. There are 2 documents included. A custom exemption and the AZDHS form. Some parents use one or the other, some include BOTH.  ****Note that the state statute does not specify a list of vaccinations you are opting your child out of, only a statement with the 3 items below included.

A.R.S. 15-873 does not specify a certain form to use, only a statement which includes:

1) the parent/guardian has reviewed information from Dept. of Health Services

2) they understand the risk and benefits of immunizations and the potential risks of nonimmunization

3) that due to their personal belief, the parent/guardian does not consent to immunization of the pupil.

Religious Exemption Form (Child Care, Pre-K etc.)   -   

If you are submitting an exemption form for your preschool, childcare aged child, utilize this link. R9-6-706  - Exemptions from Immunizations | State Regulations: A child is exempt from applicable immunization requirements for religious beliefs as backed in ARS § 36-883(C) if the child's parent submits a religious exemption.

Medical Exemption Form (K-12, pre-school, child care)

If you need to submit an exemption form for school, pre-school, child care, headstart etc, utilize this form. R9-6-706 - Exemption from Immunizations | State Regulations: A child is exempt from applicable immunization requirements as back in ARS§15-873(A)(2) if the child's parent submits to a school or child care a certification of medical exemption from immunization. It must be signed by a physician or registered nurse practitioner.  Important to note: (Under Arizona Statute, "physician" means a person licensed pursuant to Title 32, Chapter 13, 17 or 29 [1] OR a person licensed to practice allopathic or osetopathic medicine under the laws of another state or country.   Chapter 13:  Medical doctor/medical surgeon. Chapter 17: osteopath doctor/surgeon. Title 29: Homeopathic doctor.   In conclusion, this means the following can sign off on a medical exemption: Nurse Practitioner OR medical doctor (MD), osteopathic doctor (DO), Homeopathic Doctor (licensed)[1]  within the state of Arizona, another state, or another country. ASIIS Opt-Out form(ASIIS is the AZ state vaccine tracking system)
ASIIS Opt-Out Form; Adult
ASIIS Opt-Out Form; Minor

Utilize these forms to opt yourself or your child from the "Arizona State Immunization Information System (ASIIS). This will prevent health care providers, day care centers, schools/colleges from having access to vaccinations in ASIIS. Take note you may want to keep personal records on file with yourself.

Mask Notice for Public/Charter schools in Arizona

(CHILD IN SCHOOL)

Serving a notice is the first step needed when a school is attempting to mandate masks in a public or charter school in Arizona. Sending the notice certified mail is the better option. 

Send to: Superintendent, Principal, Governing Board members and any staff that has communicated these "requirements". Any person can provide a notice to a school. You do not have to be a parent of a child there. If you learn of a school pushing these draconian measures, even if you don't have a child in school, use these notices.

Notice for Public/Charter Schools in Arizona re: Parents Bill of Rights Signage on Website

On June 5th, 2020, SB1061 was passed & signed by Governor Doug Ducey establishing ARS §15-249.16 which requires ALL public/charter school to post the Parental Rights Handbook in a PROMINENT location on their district website. If you find a school is no in accordance, this notice can be applicable. Any person can provide a notice. Send to: District Superintendent & Governing Board. Certified mail is the best option to ensure the party desired receives it.

How to submit a FOIA request (Freedom of Information Act)

Requesting Public Information from AZ Dept. of Education:   I have used this to gather information on programs, email correspondence with ADE staff, superintendents, schools etc in regard to a specific topic/issue. If you want to find out if there is behind the scenes stuff going on with your schools, funding, masks, covid jabs, programs, funding etc... FOIA them and start there. All correspondence, aside from FERPA & attorney privileges is public information. I HIGHLY recommend parents start using this, find out WHAT is going on and take back control!
